This contract been The Client and The Sweat Lab is in force for 12 months from the Contract Start Date, with automatic renewal for subsequent months. You do have the option to put your membership on hold once a year for up to 60 days.

Agreement to Pay Recurring Fees

The Client agrees to pay The Sweat Lab recurring fees as follows: $130/Month via Client Authorized Automatic Credit Card Payment in monthly increments by any other means other than Client's credit card is not permitted without The Sweat Lab's authorization.

I agree to purchase the 12 Month Auto Pay Contract for 12 months, as an automatic charge to my credit card each month for a contract period of 12 months and that this membership cannot be terminated before the 12 months has concluded.*

I understand that I will be noticed if my credit card fails to authorize for any reason, that a $10 late fee will apply if I do not provide a valid credit card within 5 calendar days of the original rejection date.*

I understand that my service will be deactivated if my account becomes more than 30 calendar days late.*

I understand that if I "late cancel" (cancel after the 3 hour cancellation period or 8 hours for a 6am class) for a class, my credit card will be charged $15 per class missed. This is also applicable to the waitlist if I fail to remove myself., should I not be able to attend.*

I understand that I must be in the class 5 minutes prior to class start time, otherwise my spot can be given away to someone on the waitlist or a walk in client.*
