How did you hear about The Sweat Lab?

This contract between The Client and The Sweat Lab is in force for 1 month from the Contract Start Date, with the automatic renewal for subsequent months, unless cancelled by either The Guest or The Sweat Lab, with notification of 30 days within advance of the next billing date.

Please note if you decide to put your membership on hold, your contract will be extended for the same amount of days that you put the membership on hold for.

If The Sweat Lab is forced to place your membership on hold for unforeseen circumstances out of their control, your payments will be suspended until the studio reopens. Please note that you will still be liable for the remaining payments of your 12 month contract with your contract dates being extended accordingly.

Agreement to Pay Recurring Fees

The Client agrees to pay The Sweat Lab recurring fees as follows:

Contract Value

per Month via Client Authorized Automatic Credit Card Payment in monthly increments by any other means other than Client's credit card is not permitted without The Sweat Lab's authorization.

I agree to purchase the 1 Month Auto Pay Contract, as an automatic charge to my credit card. I understand that I need to give 30 days notice through email or in written form to cancel the month to month contract, otherwise my credit card will continue to be charged.*

I understand that I will be noticed if my credit card fails to authorize for any reason, that a $10 late fee will apply if I do not provide a valid credit card within 5 calendar days of the original rejection date.*

I understand that if I "late cancel" (cancel after the 3 hour cancellation period or 8 hours for a 6am class) for a class, my credit card will be charged $15 per class missed. This is also applicable to the waitlist if I fail to remove myself., should I not be able to attend.*
